Fee and Payment Information

Emily Laconic is a fee-for-service provider. Emily Laconic is an in-network provider for some insurance companies and considered an out-of-network provider for all medical insurance plans not accepted. Services are charged in accordance with current procedural terminology medical coding. Fees are adjusted periodically and clients will receive a 60-day notice when fees are adjusted.

Fee Schedule

  • I offer a free initial 30 minute phone or video consultation to ask questions, and determine if I am the right fit for you.
  • Psychiatric diagnostic intake assessment 90 minutes - $250.00
  • Psychotherapy (individual) 53 minutes - $200.00
  • Psychotherapy (couple) 53 minutes -$200.00
  • Psychotherapy Prolonged (individual/couple) each additional 30 minutes - $95.00
Other Fee Schedule
  • Late Cancellation of less than 48hrs/No Show Fee – with insurance is the cost of co pay and cash payer will pay their session rate.
  • Additional Professional Fees per hour - $75.00
  • Patient Record Copying Fees per page - $0.75
  • Patient Record Copying Fee Time -$18.80

Employee Assistance Program Counseling

Services are covered in full by the vendor as part of subscriber’s employee benefit package. Patients are not responsible for any fees associated with this service.

Payment

Cash, major credit cards, health savings account funds, and flex spending account funds are acceptable forms of payment. Known amounts for deductibles, copayments and coinsurance are due on the date of services. Clients will be asked to sign a credit card payment authorization agreement to secure payment for amounts in which they are responsible for paying, and keep their credit card on file for late cancelations and no shows.

Client Bill of Rights

Consumers of psychotherapy services offered by clinical social workers licensed by the State of Minnesota also have the right to the following:

  • To expect that their therapist has met the minimal qualifications of training and experience required by state law;
  • To examine public records maintained by the Board of Social Work, which contain the credentials of a therapist;
  • To obtain a copy of the code of ethics from the State Register and Public Documents Division, Department of Administration, 117 University Ave, St. Paul, MN 55155
  • To report complaints to the MN Board of Social Work, 335 Randolph Ave Suite 245, St. Paul, MN 55102 or FL at https://flhealthsource.gov/telehealth/ or Vermont at https://sos.vermont.gov/opr/online-services/

Release of Liability Cause

Except in the case of gross negligence or malpractice, I or my representative(s) agree to fully release and hold harmless, Emily Laconic, from and against any and all claims or liability of whatsoever kind or nature arising out of or in connection with my session(s).
