Fee and Payment Information

Emily Laconic is a fee-for-service provider. Emily Laconic is considered an out-of-network provider for all medical insurance plans. Services are charged in accordance with current procedural terminology medical coding. Fees are adjusted periodically and clients will receive a 60-day notice when fees are adjusted.

Employee Assistance Program Counseling

Services are covered in full by the vendor as part of subscriber’s employee benefit package. Patients are not responsible for any fees associated with this service.

Payment

Cash, major credit cards, health savings account funds, and flex spending account funds are acceptable forms of payment. Known amounts for deductibles, copayments and coinsurance are due on the date of services. Clients will be asked to sign a credit card payment authorization agreement to secure payment for amounts in which they are responsible for paying, and keep their credit card on file for late cancellations and no shows.

Release of Liability Clause

Except in the case of gross negligence or malpractice, I or my representative(s) agree to fully release and hold harmless, Emily Laconic, from and against any and all claims or liability of whatsoever kind or nature arising out of or in connection with my session(s).
